The Schedule O in Form 990-EZ is used to provide descriptive information about the organization's operations or to respond to specific questions to the IRS with respect to a particular line and response in the Form instead of using separate attachments.
Schedule O must not be used to provide additional information about other Schedules attached to the form.
The Schedule O should also be attached by an organization if they are filing an amended return. The organization must use the Schedule to mention all the changes made in the amended return.
