Which organizations are required to complete and attach Schedule H along with its Form 990?

In general, Schedule H is filed by health organizations along with Form 990 to furnish information about the activities, policies, and community benefits provided by both of its hospitals as well as non-hospital health care facilities.
 
Schedule H must be filed by organizations that gather information from:

  1. Hospital facilities that the organization directly operates.
  2. Hospital facilities are operated by disregarded entities of which the organization is the sole member.
  3. Other health care facilities and programs of the hospital organization or any of the entities as described in either of the above points, even if provided separately from the hospital's license.
  4. Hospital facilities and other health care facilities and programs operated by any joint venture which are treated as a partnership to the extent of the hospital organization's proportionate share of the joint venture.

The Schedule H will be automatically generated if required while filing your Form 990 using the "Interview-Based" filing in Tax990. The Schedule H will also be made available to you depending upon the information you enter in your Form 990 using the "Form-Based" filing in Tax990.
